1st - Download Physical Form
Download the CAA Physical Form. CJHS athletes must use the CAA Physical Form in order to participate. This form will need to be taken to the student's physical examination and filled out by the healthcare provider. Physicals must be dated after May 30, 2022, in order to be used for the 2022-2023 school year.
2nd - Register My Athlete
Complete the Register My Athlete electronically under the school name: Coolidge High School. We are no longer accepting paper registrations for the athletic department.
All forms can be viewed and signed electronically. All additional required forms must be uploaded to your account (i.e. physical form).
All athletes are required to have medical insurance in order to participate in our athletic programs.
Please do not sign up for multiple sports in one season.

What to Expect at Practices & Games
Practices will be from 2:50 pm - 4:20 pm, Monday through Thursday.
Practices are closed to parents/spectators.
Please make sure you are on time to pick up your participating students.
Games begin at 4:00 pm unless noted otherwise.
Coaches are responsible for determining the duration of play.
Student grade checks will be done each Thursday.
Any student-athlete failing may not play until the grade is brought up to passing.
Any student-athlete failing a class will not be eligible to play in next week's game(s).
Student-athletes with discipline issues at school may not be eligible to participate in games.
Student-athletes in after-school detention will not be eligible to play that day's game.
Student-athletes must attend school for at least the final 4 periods of the day to be eligible to play in a game unless they have a doctor's note.
