Physical Packet and Athlete Handbook

CHS Athletic Department will be reverting to a paper registration process for athletics for the 2024-2025 Fiscal School Year and will no longer be using a digital registration platform. Any questions regarding this change, please contact Athletic Director Dr. Clinton White.
To pick up the Student-Athlete Handbook/Physical Packet, come see Ms. MaKenzie Blank in the front office.

Please note that all parts and pages of both packets must be completed prior to turning in to Ms. MaKenzie. Failure to complete the physical packet will result in the return of the student-athlete's packet and refusal of participation in any sport or activity until it is completed.

Student-Athletes must have insurance to participate in AIA sactioned athletics per CUSD and AIA policies. If you have any questions regarding this policy, please contact Dr. Clinton White or Ms. MaKenzie Blank for more information.

Student-Athlete Handbook
There are three signature pages on the back of this document that will need to be turned in with the physical packet prior to participation.
Parents, please keep a copy of the handbook for referencing throughout the year. It has our grade pull dates in the handbook and your help in making sure your student-athletes have great grades is appreciated.

AIA Academy Course
All CHS student athletes will have to complete the Brainbook online concussion education course as well as the Opioid course prior to participation in practice and/or competition. These courses have to be completed just once in the student's high school career.
