1st - Download Physical Form

Physical Form
Download the CAA Physical Form. CJHS athletes must use the CAA Physical Form in order to participate. This form will need to be taken to the student's physical examination and filled out by the healthcare provider. Physicals must be dated after May 30, 2022, in order to be used for the 2022-2023 school year.

Registration Process
Complete the Register My Athlete electronically under the school name: Coolidge High School. We are no longer accepting paper registrations for the athletic department.
All forms can be viewed and signed electronically. All additional required forms must be uploaded to your account (i.e. physical form).
All athletes are required to have medical insurance in order to participate in our athletic programs.
Please do not sign up for multiple sports in one season.

Follow these steps:
Register My Athlete allows parents to register their athletes for sports online. Here are some basic steps to follow when registering your athlete for the first time.

1. Create an Account
Begin by creating your account by clicking on the "Create an Account" button. After filling in the required information, the system will automatically log you in and you will be required to accept the terms of use.

2. Find Your School
Select your school, Coolidge High School, CJHS Athletes will fall under Coolidge High School when registering.

3. Add a New Athlete
You can do so by clicking the "My Athletes" tab on the left-hand side of the page or by clicking "Add Athlete" underneath the "My Athletes" tab. This only needs to be done once during your athlete's entire high school sports career at a school. The information entered here will carry over from year to year. This information includes your athlete's contact information and medical information.

4. Athlete's Profile
After you've created your athlete, you will be brought to their Profile page. This page is a summary of their info and involvement.

5. Register for a Sport
Click "+ Register for a Sport" to begin registration, you will be asked to choose which sport your athlete is registering for.

6. Your Registration Checklist
This page shows the status of your athlete's registration. You will be asked to complete several steps to complete registration, including agreeing to documents and completing the physical.

7. Physicals
Physical Examination documents should be completed by the parents (or medical professionals as needed). Parents have the ability to upload these physical documents to the system. Uploaded documents will need to be verified by the admin at the school prior to being accepted as complete. (If the document uploaded is rejected for any reason, the parent will receive an email with the rejection reason. After the error has been corrected, parents will be able to re-upload the document for verification.)

8. Complete Registration
Your registration is complete once all items on the checklist have been completed.

9. After Registration
After registration is complete, you can login at any time to view the status of your athlete and their participation on the team.

Additional Athletes
Under the same account, repeat steps 3-9 to register additional athletes.

Future Seasons and Years
Once your athlete has been added to your account, you only need to follow steps 5-8 to register them for another sport.
What to Expect at Practices & Games

Practices will be from 2:50 pm - 4:20 pm, Monday through Thursday.
Practices are closed to parents/spectators.
Please make sure you are on time to pick up your participating students.
Games begin at 4:00 pm unless noted otherwise.
Coaches are responsible for determining the duration of play.
Student Eligibility

Student grade checks will be done each Thursday.
Any student-athlete failing may not play until the grade is brought up to passing.
Any student-athlete failing a class will not be eligible to play in next week's game(s).
Student-athletes with discipline issues at school may not be eligible to participate in games.
Student-athletes in after-school detention will not be eligible to play that day's game.
Student-athletes must attend school for at least the final 4 periods of the day to be eligible to play in a game unless they have a doctor's note.
