Download Physical Form

Download the CAA Physical Form. CJHS athletes must use the CAA Physical Form in order to participate. This form will need to be taken to the student's physical examination and filled out by the healthcare provider. Physicals must be dated after May 30, 2024, in order to be used for the 2024-2025 school year.
What to Expect at Practices & Games
-
Practices will be from 2:50 pm - 4:45 pm, Monday through Thursday.
Practices are closed to parents/spectators.
Please make sure you are on time to pick up your participating students.
Games begin at 4:00 pm unless noted otherwise.
Coaches are responsible for determining the duration of play.
Student Eligibility

Student grade checks will be done each Thursday.
Any student-athlete failing may not play until the grade is brought up to passing.
Any student-athlete failing a class will not be eligible to play in next week's game(s).
Any student-athlete ineligible for 2 weeks (nonconsecutive) will be required to attend afterschool tutoring.
Student-athletes with discipline issues at school may not be eligible to participate in games.
Student-athletes in after-school detention will not be eligible to play that day's game.
Student-athletes must attend school for at least the final 7 periods of the day to be eligible to play in a game unless they have a doctor's note.
